Registration Forms and Contact Information

I WANT TO GO!! WHAT DO I DO? - The forms provided below can be printed and duplicated. An application and the attached medical release must be filled out for each young person and each sponsor attending the weekend. You will need Microsoft Word to view and print the documents. If you need an application sent to you, contact your youth leaders at your parish or contact Kim Smith, the Diocesan Youth Coordinator at ksmith@episcopalatlanta.org to ask for an application.

What to Bring to New Beginnings  

• Sleeping Bag
• Pillow
• Comfortable clothes (it’s not a fashion show!)
• Extra pair of shoes
• Personal toiletries
• Towels
• Sweater or light jacket
• Package of cookies or snack food for breaks (turned in during registration for snack breaks during the weekend)
• Medications (for sponsors to distribute)

What NOT to Bring to New Beginnings  

• Electronic toys (Gameboys, CD Players, etc.)
• Alarm clocks
• MP3 Players
• Cellphones
• Watches
• Anything of great value (e.g. good jewelry)
